Classification of the Drug: Loop Diuretics
Mechanism of Action:
It inhibits the water reabsorption by blocking the sodium, potassium, and chloride in the ascending loop of Henle. The filtered fluid ultimately increases the urine output. The onset of action for oral administration is one hour and lasts for 6-8 hours. the onset of action for injection is five minutes.
Recommended Dose and Route of Administration:
For PO: Adults - 20-80mg /day initially and increased to 20-40mg q8h.
Children - 2mg/kg initially increased to 1-2mg/kg q8h.
Neonates - 1-4mg/kg/dose 1-2 times.
For IV
Adults - 20 - 40mg initially and increased to 20mg every 1-2 hr and maintain dose may give q6h or q12h.
For Continuous infusion- 0.1mg/kg/hr to a maximum of 0.4mg/kg/hr.
Children: 1-2 mg/kg/dose q6h or q12h
For continuous infusion - 0.05mg/kg/hr.
Neonate: 1-2 mg/kg/dose q12h or q24h.
