Serious mental illness requires treatment to he done in hospitals. This may affect a child or an individual wrthout any age limit. Delusions and hallucianations often affect the general life of the individual.

The following are the main components of mental health consumer- Consumer and Carer Participation:

Assessment: Assesseing the health of the patient is considered as the most important component of mental health consumer-case record.

Treatment plan: The treatment plan that is carried on for the individual must be clearly mentioned. Any person who reads the plan and assessment must understand it. Hence, it is the most Important component of mental health consumer- case record.

Progress notes: The progress of the patent's health must he monitored and must be recorded.

Discharge summary: The discharge summary of the patient must be clear and It must report the full health condition of the patient right from the beginning of the admission of the patient.

Alter care plans: The care that must be given to the patients must be explained clearly to the caretakers of the patient.
