1 .Tab.Vancomycin 500 mg PO q 12 hourly × 7 days  

Available dose is Vancomycin 500 mg, so one tablet to be taken every 12 hourly for 7 days .You can take the tablet with or without food .Even if you miss the dose try to take it as soon as you remember.Do not take two doses to make up the missed dose.

2.Tab Xanax 0.5 mg i-ii q 6 hourly prn.

Available dose is .25 mg , so 2 tablets can take at a time prn (means if necessary) for anxiety. i -ii means 1 - 2 doses .Xanax can be habit forming, so take the drug only if needed.

3.Augmentin 750 mg susp. QID × 10 days .

Available dose is Augmentin 250 mg /5 ml, we need to calculate how much mg in 1 ml . Here 5 ml contains 250 mg . So 1 ml= 250 ÷ 5 = 50 mg. 1 ml contains 50 mg. Now to calculate how much ml is required for 750 mg.

ie, 750 mg÷ 50 mg= 15 .So,15 ml is required for 750 mg.Augmentin suspension 15 ml every 6 hourly for 10 days .QID means 4 times a day.Shake the suspension before use.

4.Prozac 30 mg susp.po qd in am and hs.

Available dose is 20 mg per 5 ml ,so 1 ml is equal to 20 ÷ 5 = 4 . 1 ml contains 4 mg.To calculate how much ml is required for 30 mg ,ie, 30÷ 4 = 7.5, so 7.5 ml is required for 30 mg.Prozac suspension 7.5 ml every day in am and bed time .qd means every day.hs means at bedtime. Should not stop this medication without consulting your physician.
