Population Health: As a result of the Patient Protection and Affordable Care Act, hospitals are increasingly being held accountable for the health of patients after they have been discharged from the hospital. The CEO of Sandy Creek Community Hospital asks you to join her in a meeting with the CEO of Green Acres Retirement Community located about a mile from the hospital. Green Acres offers a continuum of care to include rehabilitation services, assisted living and skilled nursing care.
