2. information that the patient had but the medical record did not contain

3. information in the medical record that the patient did not recall

4. information that was neither in the patient's recollection nor in the medical record

Okay so for making this cross table what we did is that we broke down the conditions is two different situations which are information with the patient and information on the medical record.

for number 1 category we have the patients information matched the medical record and this is why I have written yes for both the situations that the information with the patient and the information on the medical record stating yes implies category 1.

for category 2 the information that the patient had but the medical record did not contain that is yes for the first situation and no for the second situation implies category 2.

For category 3 the information in the medical record that the patient did not recall that is no for first situation and yes for the second situation implies category 3.

For category 4 the information was neither in the patient's recollection nor in the medical record that is no for  both the situations implying category 4.

This is how the cross table is drawn on paper.

Now to draw this cross table on your ms excel 2007 go to insert and on the top corner Left hand side you will see the option pivot table, choose the pivot table and a pop up box will appear asking you to select the information (it might be selected already) and then press ok. Then on the right side, drag the information with the patient below in the row label and information on the medical record in the column label and then choices in the value label and your data table will appear just like mine on the right hand side of the spreadsheet(pasted above)
