Steps
Identify “problem-focused trigger” or “knowledge-focused trigger” that indicate a practice change.
Determine whether the “trigger” is a healthcare organization priority.
Make a team topic and team will search and synthesize literature related to the topic.
Evaluate the availability and merit of evidence. If evidence availability and merit are lacking, conduct research.
If reliable evidence is available, make the practice change.
. If change is successful, disseminate findings within the system and implement recommended change into practice
