A 65-year-old man is seen in the preoperative
clinic 1 week before a scheduled radical prostatectomy. He has been
diagnosed with femoral deep vein thrombosis (DVT) following a
complaint of calf soreness.
What is a radical prostatectomy?
What would be a reason for this procedure (i.e.
diagnostic, curative, restorative, etc.)
What is the level of urgency, degree of risk, and
extent of urgency for this procedure? (i.e.
emergency)
What possible treatment would you anticipate being
ordered for the treatment of DVT? (Discuss 2)
What are risk factors that contribute to the
development of DVT? (Name 3)
1)Radical prostatectomy is defined as the process of removal of entire prostate gland. In this process the surgeon removes the whole prostate gland .
2).Diagnostic procedure are used to find the cause (Biopsy). Curative procedure helps to identify and clear the underlying problem and protect from getting the condition worse in future (appendectomy). Restorative procedure are used to reconstruct the structure of the lost part (cosmetic surgery)
In this It is a curative procedure, is used . It normally is done in cases with BPH, other benign tumors or prostate cancer. The treatment includes the removeal of the prostate gland before tumor or cancer spread to other organs.
3) The degree of urgency in radical prostatectomy is mainly depended upon the type of cancer the patient have ,if the patient have a benign tumor then we can delay but if it is cancerous and spreading rapidly then we should be in hurry.
There is always the risk involved in this procedure because the prostate gland is surrounded by many pelvic organs, blood vessels, and nerves.
4)DVT (Deep vein thrombosis) can be treated by using thrombolytic agents, which helps to dissolves the blood clots otherwise surgery is the option.Future blood clots can also prevented by using blood thinners like apixaban, dabigatran, fondaparinux, warfarin .
Low molecular weight heparin injection can also be given if needed and has to be stopped before 72 hours of surgery .The effect of medication lasts for about 24 hours if it is given in the injection form
5)The potential risk factors includes
Obesity and overwieght.
Family history of DVT.
An injury to the deep vein.
Fracture of pelvis ,hip and lower extremity.
