1)from the above scenario ,Mr. Rob is 84 years old person considered as geriatric person. Geriatric population is considered as the most vulnerable group of population. Provision of preventive, promotive, supportive and rehabilitation services is necessary to this group of population. This is one of the key responsibilities of community health nurse. When providing care to this patient all aspect of health should consider that include
Physical aspect:nurse should consider physical status such as daily living activities, functioning of all senses such as vision, hearing, taste, smell and touch. Other considering factors are changes during elder patient including chronic disease, comorbidities and physical disabilities .loss of memory is another problem of elder person, this problem also take into consideration. In physical aspect,the health care personal also consider the food and nutritional requirement, accessibility of health care services.
Psychological factors :while providing health care services to the elder person we should take consider the psychological aspect of person. the psychological aspect include, emotional concerns, mental stress, isolation, anxiety and depression.
2)Explannation for planning process is essential to create awareness regarding planning process and participation of patient also. The process of planning include
:setting of goal :settings of goal and objectives is the first step for planning process. This involve what is the end result of giving health care to the patient.
:develop planning area:in this step we should find out the problem area ,and plan care based on the need of patient.
:consider limitations :there is lot of limitations to the provision of health care services. Identification of these limitation help to adopt alternative method to over come this limitations.
:decide planning period:we should discuss the period of provision of health care services .Establishment of period of care is help to tackle the goal as early as possible.
:integration of health care plan :integrate all health care services is essential for provision of good health care service .and finally evaluate the care planning.
3)the home care based services offered lot of service include transportation ,house hold maintenance, personal care, mental health care ,and health care services. In the case of Mr. Rob the two essential care is personal care and house hold maintenance. In personal care the health care personal help the elder person in activities of daily living ,meal planning based on their health status.
The services in household maintenance include help the person in laundry, shopping, gardening and house keeping.
4)i think Mr. Rob is not willing to individualised plan. Because he is able to do his self care activities independently, and he is happy to stay in own home. While planning the individualized care we should consider goal, concept and what is important to them. In the case Mr. Rob he is interested in doing their self care activities ,house hold activities and he is interested in staying his home for long periods .
5)as a health care personal we can help the individual to select most appropriate health care services. First we should evaluate the goal and what is important to them. Assess the needs for that person and help them to choose correct plan.for helping the individual we make positive interactions with the individual and make effective communication is necessary. Details explain the different health care plan and help them to choose best plan based on their need.
6) The persons involved when planning of individualized care is health care personal (community health nurse or voluntary worker) who provide care and the individual(Mr.Rob) who receive care and community .
7)Mr.Rob is living in a society so individualized care for Mr. Rob should inform to her friends, neighbors and community, so participation from that people help to easy provision of care.
8)it is necessary to liate with assessor, because the assessors know more about the individual,before planning care for an individual we should collect information from assessor and this will help to planning of good care and recruitment. Location is good for planning need based care for an individual.
