You work on a medical/surgical floor at a county medical center. There is a patient on your floor who had a stroke, and his wife is his power of attorney. The patient’s stroke was mild and his cognitive functions were not affected by the stroke (he is oriented times 3). He has been on the unit for 3 weeks. Early in his hospitalization his hemoglobin was found to be 6.0g. The patient’s wife is a Jehovah’s Witness; however, the patient is not. The patient’s wife is the strong hand in their relationship. She insisted that the patient should not receive blood products. The patient is on the fence with the decision to receive blood products. His wife would not allow the doctors to speak with him without her being present. The patient is not opposed to his wife being continuously present. The patient’s well-being is in danger because his hemoglobin continues to drop, and the iron tablets and other treatments are not much of a benefit. Use the resolved method to decide how the medical staff should treat this patient.
Ans:- As a nurse we need to understand the feelings of patient and patient 's family. Patient' s hospitalization is not pleasant thing for any individual in family. And sometimes it affects in the treatment and makes not deliver sufficient therapeutic care for the patient. In order to avoid family crisis, it is also important to estimate all the needs of family ,which has been categorised as needs of knowlege, emotional needs and personal needs.
Needs of knowlege - many families have inadequate knowlege concerning the provision of effective care for the patient. Needs of knowlege related to the concerning the progress of patient's condition,treatment,programe of nursing care so that the information provided by hspital staff to the families should be reliable and sincere.It is very important to make them understand which is helped in treatment.some of the major needs of knowlege, essential to family are-their daily informing from the doctor about progress of the patient
- informing about diagnostic and therapeutic care.etc.
Emotional needs- emotional needs of the family aim that the improvement of the therapeutic programme and communication with hspital staff and medical staff. Family should express their feelings to staff and to feel that staff concerns about their patient, and to feel that there is a hope for the patient recovery
Personal needs- According to the relative studies, personal needs of the family have the smallest interest.
By the given queztjon situation also, same thing we should make the patient'wife understand what we need for her husband treatment. So full of the above knowlege and realtionship should make to deliver to the wife. We should support her feelings.
