1.Nursing diagnosis(cognitive domain):knowledge deficit related to post natal care as characterised by preoccupied,disengaged.
*Expected outcome - mother will gain sufficient knowledge on newborn care.
*plan-educate the mother regarding the caring of the newborn like,giving feeding,providing warmth and attending for change of diapers etc.
assist mother in taking care of new born util she gets confidence in attending child.

2.Nursing diagnosis(affective domain) :Disturbed mood process related to early motherhood as characterised by irritation on newborn.
*Expected outcome - Maintain normal mood and feelings towards the newborn.
*plan - teach the mother regarding the mother child relationship.
explain the importance of mother in taking care of a newborn likefeeding,changing cloths etc.
encourage client to maintain the mother -and -child bond to promote good development of the child.

3.Nursing diagnosis(psychomotor domain):Ineffective role performance related to early motherhood as characterised by inattentive to newborn needs.
*Expected outcome - mother will participate in attending the needs of the newborn.
*plan- assist the mother to attend the needs of the newborn.
teach to respond to the cry of the child as it may be because of hungry or doscomfort.
