THE GENESIS OF IHS

  • congress established the IHS division of diabetes treatment and prevention in the year 1979 in response to the emerging epidemic of diabetes in American Indian and Alaska native communities.Formerly called the IHS as NATIONAL DIABETES PROGRAMME.

The mission of IHS is develops ,documents and sustains the IHS clinical and public health efforts to treat and prevent diabetes in Amercan Indian and Alaska natives.Although it takes many years to turn around an epidemic like diabetes,this is happening in American Indian communities.,with significant improvements in childhood obesity,diabetes prevalence and diabetes related kidney failure(diabetic nephropathy).

IHS provides informations and resources to strengthen clinical,public health,and community approaches to diabetes treatment and prevention throughout the united states

.The division also plays a central role in managing and supporting the special diabetes program ,which includes:

  • translating and disseminating the latest science to Indian health services ,tribal and urban indian health programmes across the country.
  • providing training on diabetes science and SDPI program management
  • facilitating the sharing of information and expertise among health care professionals and tribal communities
  • supporting grant program efforts to use best practices in diabetes treatment AND prevention.
  • providing essential clinical data for program planning and improvement through the diabetes care and outcomes audit.

TYPE 2 DM Are intended to provide guidance to clinicians and educators as they care for American Indian and Alaska natives people who have or are at risk for type 2 DM.

THE RECOMMENDATIONS INCLUDE

COMPONENT CARE /TEST/SCREENING FREQUENCY/WHICH PATIENTS(AT DIAGNOSIS=WHEN DIABETES IS DIAGNOSED)
GENERAL RECOMMENDATIONS

1)perform diabetes focussed visit

2)review care plan[assessment,goals,strength,barriers]

3)assess nutrition,physical activity,BMI, and growth in youth.

AUTONOMIC NEUROPATHY assess central vascular system,GI system,and sexual function at diagnosis and annually
BEHAVIOURAL HEALTH assess emotional health(depression) at diagnosis and annually
EYE CARE retinal imaging,by ophthalmologist or optometrist at diagnosis and annually
FOOT CARE

visual inspection of bare feet,

perform comprehensive lower extremity /foot exam

each visit,stress daily self exam

at diagnosis and annually

GLYCEMIC CONTROL check A1C,ask about medication adherence,hypoglycemia every3-6 months and each visit
KIDNEY CARE check serum creatinine,estimated GFR, at diagnosis and annually
NUTRITION provide basic nutrition education and support. at diagnosis,atleat annually or more as needed
ORAL CARE Inspection of gums and teeth,dental exam by dental professional at diagnosis and at least annually
